Group requests by section or timestamp. For each, preserve the original request and write a clear editing action. Separate confirmed changes, unclear requests and conflicts that need a client decision. Identify possible scope changes without assuming an extra charge. Do not invent timecodes or claim to have watched footage that I have not supplied.
